Objective: The unit of study: one concept, in one direction, for one deck, with its schedule.

Files:

Create: lib/first_thousand_words/study/card.ex

Steps:

Attributes: direction (:recognise | :produce | :listen, default :recognise),
state (:new | :learning | :review | :suspended), interval_days, ease,
reps, lapses, due_at, last_reviewed_at, suspended_at.
belongs_to :deck, belongs_to :concept.
Unique on (deck_id, concept_id, direction), so deck regeneration is safe.
Index on (deck_id, state, due_at) — this is the hot query.

Verify: in IEx, build a card, apply Scheduler.review/3, confirm fields advance.

Note: keep scheduling state on the card and the history in Study.Review. Storing only a
counter loses the ability to recompute or to change algorithms later.
