Objective: A card failed 8 times is not being learned by repetition; repeating it is wasted
time.

Files:

  • Modify: scheduler.ex, card.ex, practice_live.ex

Steps:

  1. Threshold at lapses >= 8 (configurable per deck).
  2. Surface a leech list; offer suspend, reset, or "add a mnemonic" for the card.
  3. Exclude suspended cards from the queue by default.

Verify: 8 lapses on a card makes it appear in the leech list and out of the standard queue.
